This Privacy Policy explains how Anthem Enterprise, LLC (“Anthem,” “we,” “us”) collects and uses information through anthem-enterprise.net (the “Site”). This Site is for business inquiries from physicians, practices, hospitals and healthcare facilities. We do not use this Site to collect patient health information, and we ask that you not submit any patient or medical information through our contact form.

Information you give us

When you use our contact form, we collect the information you choose to provide: your name, email address, company name, the services you’re interested in, and your message. We use it only to respond to your inquiry and to follow up about our services. We don’t sell it, rent it or share it with third parties for their own marketing.
